ONCE AGAIN ABOUT VICTIMS OF THE CHERNOBYL DISASTER チェルノブイリ原発事故の犠牲者についての再考

 Thirty-three years have passed since the accident at Reactor 4 at the Chernobyl nuclear power plant, yet the seriousness of this event for all humanity is still of concern. During the whole post-accident period, the nuclear lobby made many efforts to weaken public interest in this event. First of all, emphasis was made on the point that the tragedy had resulted in a small number of victims that included only workers of the nuclear power plant and firefighters involved in remedial actions to mitigate the consequences of the accident at the plant (31 people) and those who were permanently living in the most contaminated areas. It was concluded that the total number of Chernobyl victims who would die of cancer and leukemia throughout their life would be less than 4000 people [1]. Some attention was also paid to migrants from regions with very high soil cesium-137 contamination levels.

Representatives of official medicine had little interest in population living in areas with Cs-137 contamination levels of less than 5 Ci/km2. Increased morbidity and mortality among the population from the affected areas were not taken into account. After all, small doses of radiation from radionuclides incorporated into the body do not cause specific diseases. It is only possible to point at thyroid cancer whose incidence has risen dramatically after the Chernobyl nuclear power plant accident. Deaths of thousands of people in the areas contaminated with radionuclides were considered to be caused by natural causes.

Scientific findings showing that the entry of even relatively small amounts of Cs-137 into the body caused serious damage to vital organs (Bandazhevsky Yu.I. et al. 1995-2019) were also ignored.

First of all, one should mention cardiovascular diseases whose proportion in the general morbidity and mortality has been very high. It is possible to link heart and blood vessels damage to radiation exposure if radionuclides have been determined in organs of dead people – inhabitants of the affected areas. This was done at Gomel State Medical Institute, which I had the honor to be in charge of from the first days of its existence (November 1, 1990) until July 13, 1999 the day of my arrest. As a result of many years of work, radionuclide levels had been determined in vital organs of dead children and adults - inhabitants of the affected areas [2]; and we had found a relationship between damage to the cellular structures of myocardium of children and adults and Cs-137 [3]. At the same time, much attention was given to results of clinical laboratory studies of children living in raions with different radiocaesium soil contamination levels. Experimental studies on laboratory animals had showed metabolic disorders in the heart and vital organs under conditions of the natural entry of radionuclides into the body [4].

As a result of studies, cardiomyopathy induced by Cs-137 had been identified [5].

Special attention should be paid to congenital disorders being the best marker for endogenous and exogenous distress in the mother- fetus system. Clinical observations of dead human embryos and newborns and experimental studies on laboratory animals had clearly showed the negative impact of incorporated radionuclides on prenatal ontogeny [3, 6].

Dystrophic and necrobiotic changes in vital organs of dead embryos and newborns in the presence of Cs-137 incorporation illustrate the negative impact of the ecosystem established after the Chernobyl nuclear power plant accident in 1986. That was what the nuclear lobby was afraid of. Viewing the Chernobyl nuclear power plant accident as the largest among all accidents in the nuclear power industry, the nuclear lobby representatives each time focus on minor, in their opinion, humanitarian consequences relating to the health of people directly contacting radionuclides in the affected territory. And in this regard, dead human embryos with congenital malformations which I and my colleagues found in raions of Gomel oblast, the most affected oblast due to the Chernobyl nuclear power plant accident [3], were the best evidence base for the adverse effects of the existing radiation situation. My suspension from the position of Rector of Gomel Medical Institute led to the termination of many unique research studies dedicated to the effect of incorporated radionuclides on people living in a territory affected by the Chernobyl nuclear power plant accident, including the prenatal development. Our results show that Cs-137 incorporated naturally into the mother-embryo system contributes to multifactorial congenital malformations, i.e. malformations based on genetic defects [7].

The Institute for Congenital and Hereditary Diseases, Ministry of Health of the Republic of Belarus, directed by Prof. G. I. Lazjuk, Associate Member of the National Academy of Sciences, also assessed the effect of a radiation factor occurred as a result of the Chernobyl nuclear power plant accident on the prenatal development of human embryos. Gennady Ilyich established this unique Institute back in Soviet times, and over the years, along with his highly qualified students and colleagues he studied the morphological manifestations (congenital defects) of human genetic disorders, also in the context of consequences of the Chernobyl nuclear power plant accident. The Institute was closed down in the beginning of the 21st century, as the nuclear lobby was afraid of results of its activities.

Studies conducted at Gomel State Medical Institute allow to conclude that victims of the Chernobyl disaster are people living in areas contaminated with radionuclides and eating local foods, including “forest gifts”. The victims of this disaster should also include people who regularly consume foods containing radioactive elements of Chernobyl origin. Taking into account the fact that in the Republic of Belarus foods produced in Chernobyl-affected areas and containing huge amounts of radionuclides are sent to radiation-free areas since Soviet times, the population of these areas, especially children, should be considered as victims of the Chernobyl disaster.

Almost every inhabitant of the Republic of Belarus has experienced radiation exposure directly. So it is no surprise that there has been an increase in cardiovascular diseases and cancers due to this, including among children of the second post-Chernobyl generation.

That is why the country’s leadership should have asked for serious humanitarian aid and support of the world community in the 1990s and not refused it, as it was done. A method of assessment of radiation doses received by the population and individuals formed the basis for governmental decisions to render assistance to the population with a view to the Chernobyl nuclear power plant accident. A concept based on results of effects of external radiation exposure on humans was used. The influence of incorporated radionuclides on separate organs and systems was hardly considered. However, millions of people have suffered and are suffering currently because radioactive elements have been entering and are entering their body causing damage to vital organs and systems. These people are not recognized as victims of the Chernobyl disaster at the state level.

This is the main problem of Chernobyl and its negative impact on human health in the long term.

It is proved by results of research studies conducted within humanitarian projects of the European Commission and the Rhone- Alpes Regional Council (France) in Ukraine in 2013-2017. A  medical examination of the majority of the child population was conducted using modern instrumental and laboratory diagnostic methods in Ivankovsky and Polessky raions of Kiev oblast directly adjacent to the Chernobyl Exclusion Zone. The obligatory measurement of radionuclide concentrations was also carried out. These children belong to the second post-Chernobyl generation born and permanently living in the territories contaminated with radioactive elements.

The use of medical and genetic methods of examination allowed to identify serious abnormalities in their body being the cause of metabolic disorders under conditions of existing environmental distress (radionuclides in the ecosystem) (Bandazhevsky Yu.I., Dubovaya N.F., 2015-2019). At the same time, a number of discoveries were made with respect to metabolic associations, and the metabolism of the pituitary and thyroid hormones, calcium and phosphorus, and vitamins. At present, children and adults from these raions are being exposed to radiation due to the consumption of locally produced foods and forest gifts. It should be emphasised that radioactive elements, primarily Sr-90, are contained in large quantities in forest trees, which over time have turned into powerful sources of radiation. It still represents a threat to human health to contact with them many years after the Chernobyl nuclear power plant accident.

It was possible to show an increase in the level of homocysteine (a marker of the metabolic distress in the body) in the blood of children after fires in the Chernobyl Exclusion Zone [8].

The problem of cardiovascular diseases is one of the most significant in the  health care system. A huge number of people around the world die annually due to heart and vascular diseases. Moreover, the highest mortality rates have been reported in areas contaminated with radioactive elements due to the Chernobyl nuclear power plant accident, including among men of working age [1, 2, 3].
心血管疾患の問題は、医療体制の中でも最も重大な問題のひとつである。毎年、世界中で膨大な数の人々が、心臓や血管の病気のために死亡している。さらに、チェルノブイリ原子力発電所の事故による放射性元素で汚染された地域では、労働年齢の男性を含む最も高い死亡率が報告されている [123]
The world community first faced radiation exposure 70 years ago. Since then, representatives of the nuclear power industry have done and continue to do everything in their power to hide the negative effect on humans of radioactive elements that have polluted the biosphere of the entire earth as a result of nuclear weapons testing and technogenic catastrophes. This, in particular, is confirmed by the following facts. The research monograph by A.N. Marey, R.M. Barkhudarov, N.Ya. Novikova “Global Cs-137 fallout and the human” published by the Atomizdat Moscow Publishing House in 1974 presents a map of contamination with radioactive caesium of the territory of the Belarusian and Ukrainian Polesie in the 60s of the 20th century long before the Chernobyl nuclear power plant accident occurred in 1986 [4]. However, this book currently does not exist officially in the Russian Federation. After the accident at the Chernobyl nuclear power plant, it disappeared from many libraries in the USSR. We managed to find it only in one small library in Gomel region of the Republic of Belarus.
今から70年前、世界は初めて放射線被曝に遭遇した。以来、原子力産業の上層部は、核実験及び原子力技術の大惨事の結果として地球全体の生物圏を汚染した放射性元素の人間への負の影響を隠すため、権力を利用したあらゆる試みを継続している。このことは、特に、以下の事実によっても確認される。1974Atomizdat Moscow Publishing House発行のA.N. MareyR.M. BarkhudarovN.Ya. After the Chernobyl accident, the degree of radioactive contamination of the territories of Ukraine, the Republic of Belarus and the Russian Federation was very high [5-8]. At the same time, this did not stop the leadership of the USSR from using the meat of farm animals containing huge amounts of radioactive elements (in particular, 1.1х10-7-1.0х10-6 Cu/kg) for the production of sausage products, canned and partially prepared meat food items mixing it in a ratio of one to ten with the “normal” meat (From the text of the message delivered by the Chairman of the Agro-Industrial Committee of the USSR V.S. Murakhovskiy to the Politburo of the CPSU Central Committee).

The tragedy of the current situation lies also in the fact that the physiological effects in humans and animals that have been discovered in the last half century are difficult to separate from the radiation component, since radioactive elements are present everywhere. It is improper to try to present this phenomenon as some natural background radiation, since a number of radionuclides, including 137Cs, do not occur naturally.
現在の状況の悲劇は、放射性元素が至る所に存在するため、過去半世紀に発見された人間と動物の生理的影響を放射線の要因から分離することが難しいという事実にもある。セシウム137を含む多くの放射性核種は自然発生しないため、このような現象を何らかの自然にある環境放射線として提示しようとする試みは適切ではない。
After the Chernobyl nuclear power plant accident in 1986, the secret information about radiation was partially exposed. Due to the lack of medical personnel in the affected areas of Gomel and Mogilev regions, in 1990 it was decided to establish the Gomel State Medical Institute. And already in 1992, the Institute-based research studies began to establish an effect of the radiation factor due to the Chernobyl accident on vital systems of the human body, including the cardiovascular system in children living in Gomel and Gomel region, which according to official data is the territory with the highest contamination with radioactive elements [5-8]), as well as in other cities of the Republic of Belarus, in particular, in Grodno). Electrocardiography was chosen as a diagnostic test for the cardiac activity. All the children had their radioactive caesium levels measured. A non-selective study showed that 137Cs concentrations in the children subjected to monitoring were tens and hundreds of Bq per kilogram of body mass.
1986年のチェルノブイリ原子力発電所事故後、放射線に関する秘密情報が部分的に暴露された。影響を受けたゴメリ地域とモギレフ地域には医療従事者がいなかったため、1990年にゴメリ州立医学研究所を設立することが決定された。1992年、研究機関ベースの研究で、チェルノブイリ事故の放射線因子による人体の生命系に及ぼす影響が確立され始めた。これには、ゴメリ州とゴメリ地域に居住する子どもの心血管系が含まれるが、公式データによると、この場所は放射性元素による汚染が最も多い地域であった[58]。私たちの調査は、ベラルーシ共和国の他の都市、特にグロドノでも行われた。研究では、心臓活動の診断テストとして心電図検査が選択され、すべての子どもたちの放射性セシウムのレベルが測定された。非選択的研究では、モニタリング対象の子どものセシウム137濃度は、体重1キログラムあたり数十Bqから数百Bqであることが示された。
In addition, metabolic variables were also measured in the children. At the same time, a series of experimental studies was conducted using a natural model of incorporation of radioactive elements as part of food products into the body of laboratory animals [9-12]. A very important component of the studies, which later received wide international public response was the determination of radioactive elements in the organs of deceased children and adults that had lived in areas affected by the accident at the Chernobyl nuclear power plant [11, 12, 13]. As a result of the studies conducted, a conclusion was made about the negative effect of radionuclides incorporated into the body on the state of vital systems, including the cardiovascular system of the developing body. A relationship between incorporated 137Cs levels and frequency of electrocardiographic changes in children was also established. Pathomorphological studies of autopsy material of adults, children and experimental animals confirmed the relationship between incorporated 137Cs and cardiovascular morpho-functional changes [9-14].
さらに子どもの代謝数値も測定し、同時に、食品の一部として放射性元素を実験動物の体内に取り込むモデルを使用するという一連の実験研究を実施した[9-12]。中でも後に広く国際的世論の反応を受け、非常に重要とされた研究内容が、チェルノブイリ原子力発電所の事故の影響を受けた地域に住み、死亡した子どもと大人の臓器の放射性元素の測定であった[1112 13]。研究の結果、体内に取り込まれた放射性核種が、発達中の身体における心血管系を含む生命システムの状態に及ぼす悪影響について結論が出された。体内に取り込まれたセシウム137レベルと子どもの心電図変化の頻度との関係も確立された。成人、子ども、および実験動物の解剖病理形態学的研究により、取り込まれたセシウム137と心血管の形態機能変化との関係が確認されたのである[9-14]
The greatest irritation among defenders of nuclear energy is created by the fact that these pathological effects are caused by relatively small, from their point of view, doses of radioactive exposure. The studies on the medical consequences of the Chernobyl nuclear power plant accident that had been conducted at the Gomel Medical Institute since 1990, were unexpectedly discontinued in July 1999.
原子力産業擁護者の間で最大の問題となったのは、この病理学的影響が、原発推進側からすると比較的低レベルの放射線被曝線量によって引き起こされるという事実であった。 その後、1990年以降ゴメリ州医学研究所で実施されていたチェルノブイリ原子力発電所事故の医学的影響に関する研究は、19997月、予期せずに中断された。
And only many years later, in 2013-2017, it was possible to launch, thanks to the help of like-minded people from Europe, a large social project of the European Commission in Ukraine “Health and ecological programmes around the Chernobyl Exclusion Zone: Development, training and coordination of health-related projects”, within which new studies were performed.
それから何年もの時を経た2013年から2017年、ヨーロッパの志を同じくする人々の助力もあり、ウクライナにおける欧州委員会の大規模な社会的プロジェクトである「チェルノブイリ立入禁止区域周辺におけるエコロジーと健康プログラム:健康関連プロジェクトの開発・トレーニング・調整」の中で新しい研究が行われることとなった。
During the project, we were able to improve a system of the regular medical check-up of children from Ivankovsky and Polessky districts of Kiev region directly adjacent to the Chernobyl nuclear power plant which allowed us to identify the initial stages of pathological processes in vital organs and systems in the children. At the same time, abnormalities detected during the monitoring were analysed in the context of the structural and functional integrity of the body.
プロジェクトを進める中、チェルノブイリ原子力発電所に直接隣接するキエフ地域のイワンコフ地区とポレスコエ地区の子どもたちの定期健康診断のシステムを改善することができた。これにより、子どもにおける重要な臓器と系の病理学的プロセスの初期段階を見出すことが可能となった。同時にモニタリング中に検出された異常は、身体の構造的および機能的完全性という観点で分析された。
The state of the cardiovascular system in the children was assessed with the help of modern diagnostic systems, that enabled us to determine accurately the heart rate, blood pressure level and electrocardiographic changes.
子どもの心血管系の状態は、最新の診断システムで評価され、これにより、心拍数、血圧レベル、心電図の変化を正確に判断することができた。
With the help of modern radiometric equipment, it was possible to establish a relationship between relatively small quantities of radionuclides and changes in the vital organs in the children examined, including the cardiovascular system [15].
また、最新の放射線分析装置の使用で、比較的少量の放射性核種と心血管系を含む検査対象の子どもの重要臓器の変化との相関関係を確立することができた[15]
In order to identify the pathogenetic mechanisms of the abnormalities found, we carried out genetic analyses of folate metabolism, one of the main metabolic cycles in the human body, and measured blood levels of homocysteine, a product of the essential amino acid methionine [15-26].
また、発見された異常についての発病メカニズムを特定するため、人体の主要な代謝サイクルの1つである葉酸代謝の遺伝子解析を実施し、必須アミノ酸メチオニンの産物であるホモシステインの血中濃度を測定した[15- 26]
Taking into account the information received, it was concluded that the population living in areas contaminated with radioactive agents due to the Chernobyl nuclear power plant accident, was at high risk of developing cardiovascular diseases. In addition, the nature of these pathological processes may be different: it may either belong to stochastic effects or refer to deterministic states. We were able to show that pathological processes in the cardiovascular system, as in other vital organs and systems, began in childhood and were closely associated with metabolic abnormalities which occurred under the effect of radioactive elements incorporated into the body [27-30]. Thus, on the basis of results of long-term studies (1992-2018) we can draw a conclusion that under long-term chronic exposure radiocaesium causes cardiac effects.
得られた情報を考察すると、チェルノブイリ原子力発電所の事故による放射性物質汚染地域に居住する人口は、心血管疾患を発症するリスクが高いと結論付けられた。また、これらの病理学的プロセスの性質は異なる場合があり、確率的効果もしくは確定的効果かのいずれの場合もある。他の重要な臓器やシステムと同様、心血管系の病理学的プロセスは、小児期に始まり、体内に取り込まれた放射性元素の影響下で発生した代謝異常と密接に関連していることを示すこともできた[27-30]。これらの長期研究(19922018年)の結果に基づき、放射性セシウムへの長期に及ぶ慢性的な被曝により、心臓への影響を引き起こされるという結論を導き出すことができた。
The results obtained by us are inconsistent with the conclusion made  participants of a cross-sectional population-based study conducted by an international group of researchers from France (IRSN, Department of Cardiology Bichat Hospital, Paris) and the Russian Federation (Department of Cardiology, Bryansk Diagnostic Center) in Bryansk region, Russian Federation in 2009-2013 [31].
ところが私たち研究チームによって得られた結果は、フランスの国際研究グループである放射線防護安全研究所(IRSN)、パリ心臓病科ビシャ病院、及びロシア連邦(ブリャンスク診断センター循環器科)が2009年から2013年にロシア連邦のブリャンスク地域で実施した横断的集団ベースの研究の結論と相反している[31]
According to a text of the officially published article, on the basis of an assessment of results of examination of 17697 children aged 2.0–18.0 years these researchers did not observe an association between cardiac arrhythmia and 137Cs deposition levels in Bryansk region, Russian Federation, exposed to Chernobyl fallout. The suspected increase of cardiac arrhythmia in children exposed to Chernobyl fallout is not confirmed [31]. In this regard, on July 11, 2018, the Le Monde newspaper published an article «Les maladies cardiaques et Tchernobyl», where the main points and conclusions of the study were explained to the French readers. The conclusion of the article sounds categorically - “The study conducted allows to make a strong conclusion. Current contamination does not cause cardiac arrhythmias. And the previous opposite statements (I think it also refers to works by Yury Bandazhevsky and Galina Bandazhevskaya, as they were mentioned in the article – author’s note) were done on the basis of poorly performed observations. Thus, the main direct consequences of radioactive contamination due to the accident for children (under 18.0) remain almost 7000 cases of thyroid cancer observed in the late 1980s and early 1990s”.
公式発表された論文によると、上記研究における著者らは、チェルノブイリの放射性降下物にさらされたロシア連邦ブリャンスク地域における2.018.0歳の17,697人の子どもの検査結果の評価をしたところ、心不整脈とセシウム137沈着レベルの関連を見出さなかったという。チェルノブイリの放射性降下物に被曝した子どもの心不整脈の増加の疑いは確認されなかったとしたのである[31]。この研究に関し、2018711日、仏ル・モンド誌は、«Les maladies cardiaques et Tchernobyl(チェルノブイリにおける心疾患)»という記事を発行し、フランス人読者への説明として研究の主な論旨と結論を断定的に以下のように述べている。「実施された研究により、強力な結論を導き出せた。現在ある放射能汚染は不整脈を引き起こさない。以前行われた研究における反対の主張は(著者注:ル・モンド記事で言及されたように、ユーリー・バンダジェフスキーとガリーナ・バンダジェフスカヤ論文も参照)、不十分な観察に基づいて行われた。したがって、原子力事故による放射能汚染の主要な直接的な子どもへの影響(18.0未満)は、1980年代後半から1990年代初頭に観察された甲状腺癌のほぼ7000症例のままである。」
Due to this conclusion, I had to review the article published by the team members from IRSN and Bryansk Diagnostic Center [31].
このような結論が発表されたため、私はIRSNとブリャンスク診断センターの研究チームが発表した論文を再検討する必要性に迫られたのである[31]
A methodological part is the main part of a scientific study. In this regard, a number of principal aspects should be noted:
まず研究における方法論は、科学的研究の主要部分である。この点で、いくつかの主要な点に着目する必要がある。
2/3 of the children examined were urban children, so a selective study was conducted in Bryansk region, since children from rural areas who were in direct contact with radionuclides were not included in this study. This is understandable because it is much easier to organise the examination of urban children than to go to small localities. However, it is necessary to take into account that urban and rural residents have different socio-economic living conditions.
フランスとロシアの研究において、調査した子どもの3分の2は都市部の子どもであり、放射性核種と直接接触した農村部の子どもはこの研究に含まれておらず、ブリャンスク地域で選択的研究を実施している。人口の小さな地域に行くよりも都会の子どもたちの検査を行うほうが、はるかに容易であることはわかるが、都市部と農村部の住民の社会経済的生活条件は異なることを考慮する必要がある。
Currently, urban children in present conditions are less exposed to radiation even if they live in radiation-contaminated areas, since they have much more opportunities to consume “clean” food in terms of radiation compared to rural children. Due to poverty, rural children are constantly forced to consume locally produced food – cow’s and goat’s milk, vegetables grown on soils contaminated with radionuclides, meat of wild animals, fish from local water bodies, forest berries and mushrooms [15].
現在、都市部の子どもたちは、地方の子どもたちと比較して、放射線に「汚染されていない」食物を消費する機会がはるかに多いため、放射線に汚染された地域に住んでいても、放射線への曝露は少ない。一方、農村部の子どもたちは貧困のため、牛やヤギの牛乳、放射性核種で汚染された土壌で育てられた野菜、野生動物の肉、地元の水域の魚、森の果実、キノコなど、地元産の食料を常に消費しなければならない。
Due to the fact that forest trees are used for heating houses and cooking in rural areas, rural children have to constantly breathe air containing wood combustion products, including radioactive elements.
森林の木材は農村部で家の暖房や調理に使用されるため、農村部の子どもたちは放射性元素を含む木材の燃焼生成物を含む空気を絶えず吸わなければならない。
Therefore, high radionuclide levels in rural children are stable and consistent. A cross-sectional radiometric study among rural children will reflect more objectively the radiation expansion than among urban children, whose radionuclide levels can vary greatly depending on what food products are consumed. Therefore, even the selection of contaminated and control districts on the basis of 137Cs soil deposition makes no sense.
従って、地方の子どもたちの体内における放射性核種レベルは一貫して高いレベルにとどまっている。農村部の子どもを対象とした横断的な放射線分析研究は、都市部の子どもたちよりも、放射性物質の増加をより客観的に反映することができる。というのも都市部の子どもたちの場合は、消費される食品によって体内における放射性核種のレベルが大きく異なる可能性があるためである。従って都市部においては、セシウム137の土壌沈着に基づいた汚染地区とコントロール地区の選択でさえ意味がなくなってしまう。
Thus, the principal methodological mistake in the organisation of this study is a cross-sectional examination of mainly urban rather than rural child population.
フランスとロシアの共同研究における主要な方法論的誤りは、農村部の子どもたちではなく、主に都市部の子どもたちに対して横断的調査を行った点である。
 As a counter-argument, I can say that within the project of the European Commission we focused on results of examinations of rural children, since they are exposed to more radiation through consuming locally produced food and really reflect the effects of radiation exposure 30 years after the Chernobyl nuclear power plant accident.
反対に地方の子どもたちは、チェルノブイリ原子力発電所事故から30年、地域で生産された食品を消費し、30年間放射線に被曝され続けており、この度の欧州委員会のプロジェクトでは、地方の子どもたちの検査結果に焦点を当てた。
If we compare the work under analysis with the studies conducted at the Gomel Medical Institute in 1992-1997, one can note the following.
1992年から1997年にゴメリ医学研究所で実施された研究と現在同プロジェクトにおいて分析中の研究を比較すると、以下の点を挙げることができる。
When conducting a study aimed at identifying effects of a particular environmental factor on a particular system, in this case, the cardiovascular system, it is necessary to carefully select the groups to be examined. In particular, a comparative analysis should be made between children of the same age both within a group and between groups, who constantly and for a long time (most of time of a day) are in the similar conditions and receive the same type of meals. Currently, this is difficult to implement in the post-Soviet space. We managed to conduct our studies in the Republic of Belarus in 1992-1993, when a similar system of social life existed, in particular, pre-school and school institutions. The population also lived in quite the same socio-economic conditions.
特定の系、この場合は心血管系に対する特定の環境要因の影響を特定することを目的とした研究の場合、検査するグループを慎重に選択する必要がある。特にグループ内およびグループ間で、同年齢の子ども間で比較分析を行う必要がある。被験者の子どもたちは、常に(1日のほとんどの時間)同じ状態にあり、同じタイプの食事を摂取する必要がある。ソビエト崩壊後の現在、これを実施することは困難である。反対に1992年から1993年にかけてベラルーシ共和国では、特に幼稚園や学校施設など、社会生活において類似したシステムが存在していた期間、我々はそのような研究を行うことができた。人口もほとんど同じ社会経済的条件の中で生活していた。
For our studies, we chose pre-school institutions which were similar in terms of the number of children, buildings and diets in Gomel region and Grodno. The children aged 3.0-7.0 years, whom we examined were considered to be healthy and attended pre-school institutions. They had no bad habits, did not smoke, did not drink alcohol or use drugs, which is very important when conducting studies on the relationship between radiation exposure and the state of the cardiovascular system. Due to the socio-economic conditions existing at that time (total poverty of the post-Soviet period), during the day, children were given similar diets established by the state, and food products with the same 137Cs and 90Sr contamination levels according to the republican acceptable limits. Thus, we examined officially healthy children who were constantly in the same conditions of living and feeding and marked radiation exposure.
当時の私たちの研究では、ゴメリ地域とグロドノ地域の子どもで、数、構成、食事の点でも類似している就学前保育所を選択した。私たちが調べた3.07.0歳の子どもたちは健康であるとみなされ、保育所に通っており、子どもたちは悪い習慣がなく、喫煙も、飲酒もないし、薬物も使用していなかった。これは放射線被曝と心血管系の状態との関係を研究する際に非常に重要である。当時存在していた社会経済的条件(ソビエト崩壊後における全体的な貧困)により、日中、子どもたちは州によって確立された同レベルの食事と、ベラルーシ共和国の許容限度に従ったセシウム137とストロンチウム90に汚染された食品を与えられた。そのため、生活と摂食の状態が常に同じで、放射線被曝が著しい公式に健康な子どもを私たちは調べたのである。
The authors of the study under review used for the determination of the relationship between the radiation factor and arrhythmias urban children of different ages, from families with different socio-economic opportunities, and these factors have a different impact on the state of the cardiovascular system. It is impossible to identify the true cause of cardiovascular conditions in this situation. A child may live in excellent conditions and not being exposed to radiation, but before the study s/he may have eaten foods containing radionuclides. As a result, an ECG examination will not identify any abnormalities, while a radiological examination will detect radioactive elements in the body.
一方、今回のフランスとロシアの研究者らは、都市に住む異なる年齢の子供における放射線因子と不整脈との相関関係の決定において、心血管系の状態に影響を及ぼしうる様々な社会経済的背景を持つ家族を研究対象としている。このような状況では、心血管疾患の真の原因を特定することは不可能である。また子どもたちは、現在良好な状態で生活し、放射線に暴露されていることはなくとも、研究の前に放射性核種を含む食物を食べていた可能性がある。その結果、心電図検査では異常は特定されなくとも、放射線検査では体内放射性元素が検出されるであろう。
It is very doubtful to conclude that with such a high level of drug addiction in Russia, the children aged 12.0-18.0 years from the group examined have never smoked or used drugs. A questionnaire survey of children in most cases will not provide objective information.
またロシアでの薬物中毒の割合は高いため、調査したグループの12.0歳から18.0歳の子どもたちが、喫煙したり薬物を使用したりしたことは一度もないと考えるのは大変難しい。子どもたちへのアンケート調査は多くの場合、客観的な情報を提供してくれない。
The next problem is that the study conducted detected 137Cs radionuclides in several percents of cases of the total number of children examined. 137Cs radionuclides were not detected in 78.6 % of children from contaminated territories and 86.3 % of children from control territories.
さらなる問題は、実施された研究では、検査された子どもの総数の数パーセントのケースでしかセシウム137放射性核種が検出されなかったことである。放射性核種セシウム137は、汚染地域の子どもの78.6%で検出されず、コントロールの地域では、子どもの86.3%で検出されなかった。
Thus, the children in whom 137Cs radionuclides were detected comprise a small proportion of children examined.
このように、セシウム137の放射性核種が検出された子どもたちは、検査された子どもたちのごく一部となっている
The reason for this may lie in the fact that mainly urban children were examined. This is partly because the detection limit for children in this study was determined to be 1200 Bq per body. It was established that concentrations below the limit were not recorded. The authors ignored the detection of smaller quantities of radionuclides in the body of children and their relationship with abnormal metabolism and the state of the cardiovascular system. The studies that we conducted in 2013-2017, as part of the project of the European Commission found that there were associations between relatively small concentrations of radionuclides and a number of physiological variables [15].
この理由はおそらく、フランスとロシアの研究においては主に、都市部の子どもたちが検査されたという事実にあり、またこの研究における子どもの検出限界が一検体あたり1200 Bqとされたためであり、検出限界以下の濃度は記録されない手法がとられたためである。フランスとロシアの研究の研究執筆者らは、子どもの体内の少量の放射性核種の検出と、異常な代謝および心血管系の状態との関係を無視した。一方、欧州委員会のプロジェクトの一環として20132017年に実施した研究では、放射性核種の比較的低い濃度と多くの生理学的変数の間に関連性があることがわかった[15]
All the children were united in groups according to the level of 137Cs specific activity, without taking into accout the age. Thus, high radionuclide levels were taken into account and the age of the groups under comparison was not considered.
またフランスとロシアの研究では、すべての子どもたちは、年齢を考慮せずに、セシウム137比放射能のレベルに従ってグループ分けされた。よって、高い放射性核種レベルが考慮された一方で、比較対象のグループの年齢は考慮されなかった。
It is not correct from a methodological point of view to determine any phenomenon, including the presence of cardiac rhythm disorders, in this relatively small group in comparison with a total group. In the studies conducted at the Gomel Medical Institute during the first ten years after the Chernobyl nuclear power plant accident 137Cs radionuclides were detected in all children examined, since the whole population of Gomel region, and the whole of Belarus was exposed to incorporated radioactive elements [9-12].
全体的なグループと比較して、この比較的小さなグループで、心拍リズム障害の存在を含む現象を決定することは、方法論的な観点からは正しくない。チェルノブイリ原子力発電所事故後の最初の10年間にゴメリ医学研究所で行われた研究では、ゴメリ地域の全人口とベラルーシ全体が体内に取り込まれた放射性物質に暴露されたため、検査したすべての子どもにセシウム137が検出された [9-12]
The policy of state structures on the distribution of food products contaminated with radionuclides among all the inhabitants of the country also added to contamination of the population.
さらにすべての住民に放射性核種で汚染された食品を流通させた国家構造の政策によって、人口における汚染問題が増加した。
The following remark concerns the material presented. Different types of cardiac abnormalities, including in separate age groups, were not presented in the article under review. I think that mixing all types of arrhythmias in one group is an illustration of a non-professional attitude towards the problem. The genesis of various types of cardiac arrhythmias can be different. There are some types of arrhythmias that can be evaluated as stochastic effects. At the same time, the radiation dose received by the human body determines only the probability of their occurrence, but not their severity. Incomplete right bundle branch block detected by us in a large number of children in Gomel region belongs to such arrhythmias. We established a dose-effect relationship with respect to this very type of arrhythmia [12]. It should be noted that the frequency of this type of arrhythmias is much higher in the Republic of Belarus than in Ukraine [32]. Unfortunately, the authors did not provide statistics on this type of arrhythmias, as well as on certain types that they identified among the children examined in Bryansk region.
以下の発言は、フランスとロシアの論文において提示された研究資料に関するものである。別の年齢層を含む様々な種類の心臓異常は、この査読論文には掲載されていない。 1つのグループに全タイプの不整脈を混在させることは、この問題に対する非専門的な態度の一例である。各種の心不整脈の発生は異なる原因である可能性がある。確率的効果として評価できる不整脈にはいくつかの種類があり、同時に、人体が受ける放射線量はその発生確率のみを決定し、重症度は決定しない。ゴメリ地域の多くの子どもたちで検出された不完全右脚ブロックは、そのような不整脈に属する。私たちの研究では、このタイプの不整脈に関して、線量効果の関係を確立した[12]。このタイプの不整脈の頻度は、ウクライナよりもベラルーシ共和国ではるかに高いことに留意する必要がある[32]。残念ながら、フランスとロシアの著者らは、このタイプの不整脈、およびブリャンスク地方で調査された子どもたちの間で特定したタイプの統計を提供しなかった。
There are arrhythmias that belong to the group of deterministic effects, which suggest the presence of a certain minimum limit, below which the radiation effect is completely absent, and above which the severity of the effect depends on the dose received. Such arrhythmias can include bradycardia and tachycardia, and also sinus bradycardia and sinus tachycardia. The authors of the study came across such arrhythmias, since they indicate their codes among the material analysed: abnormalities of heart beat, R00.0 - tachycardia, unspecified; R00.1 - bradycardia, unspecified. However, statistical data on this type of arrhythmia is also absent.

A single examination of children does not enable to identify accurately cases of tachycardia and bradycardia, taking into account that they may be caused by different pathological processes.

The article under analysis discussed the issue of an effective radiation dose of 1mSv per year, but this is not correct at the the present moment.

In recent years in Russia, a level of 0.1 mSv/year is applied to employees of enterprises with links to radiation exposure [33], this level should be even smaller for the population living in affected areas.

Thus, the publication under review reflects serious violations in conducting the cross-sectional population-based study carried out by the international group of researchers from France (IRSN, Department of Cardiology Bichat Hospital, Paris) and the Russian Federation (Department of Cardiology, Bryansk Diagnostic Center) in Bryansk region, Russian Federation in 2009-2013 [31].
したがって、現在考察している2009年から2013年におけるフランス(IRSN、パリ心臓病科ビシャー病院)及びロシア連邦(ブリャンスク州にある診断センター)による国際共同グループの横断的集団研究[31]には、研究における重大な違反行為が反映されている。
The data presented throws discredit upon their reliability and significance. One can only regret that the large-scale study has no scientific value because of the gross methodological mistakes.
上記研究で提示されたデータは、信頼性と重要性において疑念を投げかけられるものである。本大規模研究には、重大な方法論的誤りがあるため、科学的価値がないと断じざるを得ない。
Organisers of such studies should have been more responsible towards the problem of consequences of technogenic catastrophes, since the health and lives of many millions of people depend on it.
このような研究の関係者は、何百万人もの人々の健康と生命が論文内容に依存しているため、原発事故という技術的大惨事の結果の問題に対してより責任を持つべきである。
The analysis of the material published allows to state that the authors of the following article made a wrongful conclusion.
